holiday request form

An accident form MUST be filled out whenever you administer any first aid.

Please give us as MUCH notice as possible when you are thinking of going on holiday.

We will not be able to guarantee your time off if we do not get enough time to cover your sessions.

We need at least FOUR weeks notice of any holiday.

Please fill out the holiday form on the staff portal and NOT agendrix.

Please do not book a holiday until your time off has been approved.
