An accident form MUST be filled out whenever you administer any first aid.


An accident is when a child has been injured.

Ensure you get the name(s) of the child or children involved.




Filling these forms out correctly will ensure you are protected from any further liabilities by following the correct procedures.

A safeguarding form will need to be filled out when and if you come across a safeguarding concern. ​

A safeguarding form should be filled out with as much detail as possible. You should include the procedure you followed and who you contacted.

You need to ensure you get the name of the child or children involved as well as any of the adults you have spoken/reported the concerns to.

Following the correct procedures will help protect you from any further liabilities.
